The Division of Occupational and Professional Licensing is here to help serve your needs whether you have a license with our office, want to get a license with our office or have an issue with one of our licensees as a consumer.

At the bottom of this page, you will find our Information or Help Request Form. You can choose to receive your information by phone, email, or virtual appointment. We will be in touch with you at a time that you list as convenient and we can hopefully serve your needs or resolve your issue in a timely manner.

Please be as detailed as possible when filling out the form below. The more information you provide to us about your specific needs the more assistance we can provide.

Note: If you have an urgent or time-sensitive issue, please refer to our contacts page for immediate assistance during regular business hours.

  1. If you select "Phone" as your chosen method, you will be called by a member of our staff during the time window selected. If we cannot reach you, a voicemail will be left with your requested information or an alternate method will be used.
  2. If you select “Email”, a member of our staff will provide you with any information relevant to your request to the email provided in the form.
  3. If you select "Virtual Appointment"  as your chosen method, as your chosen method, we will reach out to you by email with an available time in your given window. Virtual appointments are held using Google Meet. If you plan to join the appointment from a computer browser, you do not need a Google or Gmail account. If you plan to join the appointment from a mobile device, you will need to be signed in with a Google or Gmail account.
