File a Complaint - Cemetery Oversight

Download the Cemetery Oversight complaint form (Word)

The Office of Cemetery Oversight assists consumers with complaints against cemeteries, crematories, monument dealers and providers of burial goods and services. A written complaint, stating the facts upon which the complaint is based, should be addressed to the Office of Cemetery Oversight at 500 N. Calvert Street, Baltimore, Maryland 21202.

The office will present the complaint to the registrant or permit holder against whom the complaint was raised. If the cemetery, crematory, monument dealer, and/or provider of burial goods and services does not address or resolve the complaint to the satisfaction of the consumer, the parties will be invited to participate in a mediation conference. If either party elects not to participate in mediation, the director of the Office of Cemetery Oversight may recommend that the parties submit their dispute to binding arbitration. If either party is unwilling to participate in binding arbitration, the director, if warranted, may proceed with an investigation of the complaint and with disciplinary action against the registrant or permit holder.
