Use this form if you are an inspector wishing to apply or re-register to be a Third Party Qualified Elevator Inspector. Also included are the Requirements and Policy of the Safety Inspection Unit regarding registration of 3rd Party Elevator Inspectors. When submitting the application or re-registering please mail the required documents to the Division of Labor & Industry P.O. - Escalator and Moving Walk Safety Inspection and Test Form
This form is to be completed during the test and inspection of escalators and moving walks in the A17.1 code. - Vertical and Incline Lift Safety Inspection and Test Form
This form is to be completed during the one year test and inspection as well as the five year test and inspection of the applicable A18.1 equipment. - TPQEI CAT 1 and CAT 5 Inspection and Test Form
This is the report form used by Third Party Qualified Elevator Inspectors when conducting CAT 1 or CAT 5 Inspections.
